Kaiser Insurance Participants Please Read!!!

Attention Kaiser Participants: Please schedule your student-athlete’s physical to take place at least 2 weeks prior to the start of the sports season. Kaiser holds the physical form for 5-7 business days before they give it to the patient. Kaiser will give you a letter stating the physical has been completed or they will give you a copy with no doctor’s signature. In place of the doctor’s signature is a stamp that says “Completed by health information forms specialist.” This is NOT a valid physical. The physical MUST have the doctor’s signature on it.
